Distance Education proctored exams must be taken at an official testing facility. This page describes the procedures for testing on the Anchorage campus and other proctoring facilities throughout the state and how to get set up.

UAA Distance Education Courses:

All students taking DES courses must fill out the appropriate proctoring forms to set up testing at their desired location.

•  Proctor Form (Preapproved proctor sites)
  

• Use this form if your desired testing location is in the Exam Site drop-down menu.
  
  

• Alternate Proctor Form pdf (No preapproved proctor site near your location)

• Use this form if your desired testing location is not in the Exam Site drop-down menu.
  
  

• Appointment Request Form (For Scheduling at the Anchorage Campus ONLY)

• If you are taking your exam at the UAA-Distance Education Services office, please use this form to request an appointment for testing.
  
  
• Due to limited testing space we do not offer same day testing, please make sure to schedule all of your appointments in advance.
  
  
• If you are testing at a different location (i.e. KPC, Mat-Su, Eagle River), please contact that testing site to schedule your appointment(s).
  

UAF Center for Distance Education Courses:

If you are wanting to take your UAF proctored exam at the UAA Distance Education Services testing center please complete both forms below.

• UAF CDE Exam Request Form
  
  
• Requests UAF CDE to distribute exam(s) to the UAA Distance Education office for testing. 
  
  
• Failure to submit this form will result in the exam not being at the testing site when you shows up for testing.

• Designates your testing location.  Note that Anchorage residents will automatically be assigned to UAA Distance Education Services

Students who need to take exams for NON-UA Distance courses

If you are in Anchorage and needing an exam proctored that is not for a University of Alaska Distance course (UAA, UAF, UAS or Creighton University) please contact Advising and Testing with UAA at 907-786-4500 for proctoring information.  UAA Advising and Testing proctors all non-UA exams such as BYU, Rio Salado, and others.
