The Faculty Technology Center's Digital Media Services Office offers media conversion, equipment loans, video recording, and test scanning.

To get started using our services, make a request by downloading and filling out one of the sets of forms below.

Digital Media Service

Submit a Digital Media Service Request if you wish to:

• Stream video or audio on the web
• Convert video or audio from an analog format (such as VHS) to a digital format (such as a DVD/CD)
• Scan pictures, photographs, or slides and convert them to JPG or PDF format
• Copy pictures, photographs, film or slides to DVD/CD
• Duplicate DVDs/CDs

To submit a Digital Media Service Request, download and the following forms and bring them to LIB 215:

Equipment Loans

Submit an Equipment Loan Request if you wish to:

• Check out a microphone
• Check out a camcorder

To submit an Equipment Loan Request, download the following form. Then bring it to LIB 215 or submit it electronically to ayftc@uaa.alaska.edu:

Test Scanning

Submit a Test Scanning Request if you wish to:

• Scan and score exam sheets
• Request analysis reports for scanned exams
• Export exam sheet data to a file or compact disk

Video Recording

Submit a Video Recording Request if you wish to:

• Request video recording by FTC staff
• Request video editing by FTC staff

To submit a Video Recording Request, download the following forms and bring them to LIB 215:

• Video Recording Request Form
• One (1) Videotaping Consent Form for each person filmed
