Verification Checklist Step 1 Print a Copy of the Verification Worksheet - The worksheet can be found under the forms link on the Financial Aid web page
- Read through page one of the worksheet. If you leave out part of the information or documentation your financial aid will be delayed.
Step 2 Collect your financial documents - Student's Signed Federal Income Tax Form (1040, 1040A, 1040EZ or a signed copy of your Tax transcript which you can get from the IRS). Do not attach schedules, forms, worksheets
- If you are considered Dependent, we need your Parent's Signed Federal Income Tax Form (1040,
1040A, 1040EZ or a signed copy of their Tax transcript which you can get from the IRS). Do not attach schedules, forms, worksheets - If you are married, a copy of your spouse's signed Tax form (if you filed separately)
- If you filed a joint tax form and were separated or divorced at the time you completed the FAFSA we will need a copy of your W-2's with your tax form
- If you or your parents (if dependent) or you or your spouse (if independent) were active military, you will need a copy of your Leave and Earnings Statement (for the year of the tax form you are submitting) to complete the information on the Verification worksheet under untaxed income. In most cases this will be the year-end statement (usually December) for the same year as your taxes
- If you or your parents did not file taxes and were not required to file taxes, gather all earned and unearned income information, (Including Veteran's non-educational benefits; housing, food and other allowances paid to you (i.e. military, clergy). You will need this to complete the information on the verification worksheet
- Child Support payments you or your parents received or paid for ALL children in your household
Step 3 Carefully complete all sections of the Verification Worksheet that apply to you - Do not leave anything blank, enter zero
- Sign and date the Worksheet
- If you are dependent, your parent(s) need to sign as well
Step 4 Fax, E-mail, Mail or drop off your completed Verification paperwork. - The fax number, address and E-mail are listed at the top of the Verification Worksheet. Please use a cover sheet with your fax and reference your student ID and the reason you are submitting the documentation (Verification paperwork).
Step 5 Monitor your Financial Aid Status on UAonline
Step 6 Monitor your UAA E-mail account - Outstanding requirements reminders are periodically emailed to your student account
