Declaration of a Minor

Apply for Graduation

Please read the UAA Catalog for policy regarding academic minors. A minor may only be issued simultaneously with a baccalaureate degree. Students must declare minors no later than the deadline to submit an Application for Graduation and must include the minor on the Application for Graduation.

In order for "Minor in Gerontology" to appear on your transcript your must 1.)  complete the Declaration of Minor Form above and submit it to Enrollment  Services 2.) submit the same form to the Director of the Gerontology Minor Program. 3.)  meet with the director of the gerontology minor program to document that you have taken the required classes and complete petitions for appropriate classes and practicums 4.) You must also declare, "gerontology minor" when you apply for graduation. 

Note: You will not be able to save this electronic version but you will be able to print it out. One copy must be submitted to enrollment services and one copy must be submitted to
