         To change degree programs, you must formally apply for admission to the new program through the Office of Admissions and pay the appropriate fee. This applies both to changes between schools/colleges and to different degrees with the same school or college (such as a change from the MFA in Creative Writing to the MA in English). 

• Graduate Academic Petition (new form)

• Reinstatement to Graduate Degree Status

• Graduation Requirement Report (GRR/Graduate) (new form)
        Use for: Written Comprehensive Exam, Oral Comprehensive Exam, Thesis, and Graduate Project.
  
  
• Master's Thesis Submission Form
         Submit the Thesis Submittal form with two final copies of your thesis to the Graduate School. Pay binding and microfilming fees at the UC Cashiers windows.
  
• UMI/ProQuest Micro-filming Contract Forms
         Bring completed UMI/ProQuest forms to Graduate School with the Thesis Submittal form. Complete pages 4 and 5. If you would like your thesis copyrighted, or request extra bound copies, complete pages 6 and 7.
