Many of the links below will lead you to the State-wide Human Resources website, so it's important to remember to download the Social Security form 1945 before leaving this page. Prior to presenting to the candidate for signature it is recommended that you have your HRS Consultant review the draft appointment letter.
IMPORTANT: Every new employee who is eligible for UA retirement benefits MUST also sign the Social Security Form SSA-1945. This form should signed by the employee at the time they sign the appointment letter. The original form must be returned to Human Resource Services. If the SSA-1945 is required it will be noted below.
