The Agency for Healthcare Research and Quality (AHRQ) has awarded the Institute for Circumpolar Health Studies (ICHS) funding to conduct systematic studies of on-going, successful efforts to transform the delivery of primary care in Alaska. ICHS is partnering with Southcentral Foundation in this endeavor. This two year award assesses the impact of the Patient-Centered Medical Home (PCMH) model on the nature and quality of health care delivery for Alaska Native patients at the largest Alaska Native-owned and managed primary care center in the state. The study employs a mixed methods approach with the aim of assessing patient and provider experiences and satisfaction, as well as steps taken to overcome challenges and achieve the PCMH transformation. |
