Graduate School Forms

Instructions:

(to avoid processing delays follow the steps below)

  1. Download the form you need  and complete it electronically or print the information clearly and legibly.
  2. Complete it manually or electronically, then forward for signatures.
  3. Most forms allow electronic signatures: (Exception is the Thesis Approval Form which requires ORIGINAL signatures.) You must obtain all signatures from the following:
    • Your advisor (and committee, if required)
    • Your departmental director/chair
    • Your college dean
  4. Once you have completed the form and obtained all signatures, retain a copy for your records. For most forms, your program or college will forward to the Graduate School. Submission options are:
    • Use campus mail and send to: Graduate School, BOC3, suite 368, OR
    • Scan and email to Elisa Mattison, Graduate School Director (preferred), OR
    • Hand deliver to: 1901 Bragaw Street, suite 368.

    Review the admission requirements for your desired program. Submit Letters of Recommendation, resumes, statement of goals to the Office of Admissions.

    All other programs have official Graduate Studies Plans in DegreeWorks.

     

  • English Proficiency Examination Exemption Request Form

    AS OF FALL 2017 UAA NO LONGER OFFERS ESL CLASSES

    All international students must show that they have obtained a certain level of English Proficiency.

    For Master and Doctoral Degree Applicants:

    Minimum TOEFL Score: 79 iBT

    Minimum IELTS Score: 6.5 Overall Band

    An English Proficiency Examination Exemption Request Form may be submitted if the student:

    • Is a Native English Language Speaker
    • Has attended a regionally accredited U.S. college or university, and has
      • received a "C" or better in a course equivalent to UAA's English 111 course, or
      • obtained a Bachelor's or Master's Degree.

    UAA does not offer conditional admission due to incomplete or low English Proficiency Test Score.

    Deviations from academic catalog policies or requirements must be approved by submitting this form. Petitions must be approved by all signers to move forward to the next level. The Graduate School Dean has final approval on petition. Please read the catalog before you submit a petition.

  • Add/Drop Courses

    Must have all appropriate signature before submitting by the semester deadline.

  • Change of Major or Emphasis Area

    Change of Major or Emphasis Area

    To change majors or emphasis area in within the same degree and within the same college.

  • Annual Report of Graduate Student Progress

    Annual Report of Graduate Student Progress

    Required annually for all students awarded Research, Teaching, and Services Assistantships or those programs wishing to document student progress as part of the academic process.

  • Project Forms

    Change in Graduate Advisory Committee

    Required for all students completing projects who are revising their committee. If new committee, use Project Topic, Research Compliance, and Committee Form below.  

    Report on Project Defense

    Required for all students who are completing a project instead of a thesis or dissertation.

    Project Approval Form (Master’s)

    Replaces the traditional Signature Page for the master’s projects.  REQUIRED FOR ALL PROGRAMS REQUIRING A PROJECT. This form is used to signify final approval of your project. Your program may require a traditional signature page, consult your advisor to find out if this applies to you.

    Project Approval Form (Nursing only)

    REQUIRED FOR ALL NURSING STUDENTS. This form is used to signify final approval of your project. The School of Nursing may also require a traditional signature page, consult your advisor to find out if this applies to you.

    Institutional Repository Waiver Form (Projects only) 

    Required as part of your project submission packet to the UA Institutional Repository. Complete and give to your program project liaison.

    Project Topic, Research Compliance, and Committee Form

    Required of ALL graduate students completing a final research project. If research categories are NOT applicable to your project, then check “NO” for all categories. This form must be submitted. 

  • Report on Clinical Portfolio Form

    Report on Clinical Portfolio

    Required by students in the MS Clinical Psychology program.

  • Comprehensive Exam Report Form

    Report on Comprehensive Exam

    Required for programs that require a written or oral comprehensive exam.

    Request for Outside Examiner

    MANDATORY FOR ALL DOCTORAL ORAL COMPREHENSIVE EXAMS (except for qualifying exams) and dissertation or doctoral project defenses. Requests must be submitted to the Graduate School at least two weeks (ten working days) in advance of the exam. Please allow 15 days if scheduled during the summer semester.

  • Thesis/Dissertation Forms

    Required for all students completing theses or a dissertation and are revising their committee. If new committee, use Thesis Topic, Research Compliance, and Committee Form below.

    Required for all students who are completing a thesis or dissertation.

    Thesis/Dissertation Approval Form

  • Request for Temporary Leave of Absence

    Request for Temporary Leave of Absence

    Required to temporarily suspend your studies while admitted to a graduate degree program. If you are not “continuously registered” (see catalog policy) or submit a Request for Leave of Absence, you will be removed from degree-seeking status and will have to be reinstated before resuming your studies.

  • Reinstatement to Graduate Degree-Seeking Status

    Reinstatement to Graduate Degree-Seeking Status

    Submit if you have been removed from your program for failure to undertake continuous enrollment and/or not make continuous progress.

    • You must have also been in good standing (overall GPA and most recent semester GSP is 3.0 or higher.
    • Your last enrollment was less than two years ago. For periods over two years, you must reapply for admission to your program.
  • Graduate Assistant/WWAMI Health Insurance Enrollment Form

    Health Insurance Enrollment Form  (please fully read instructions below)

    Graduate Assistants/Fellowships: Required for mandatory health insurance enrollment.  The form must be completed no later than the tuition and fee payment deadline each semester. Under Coverage Period enter only the semester(s) listed on your signed contract letter.  Graduate Teaching Assistants are personally responsible for completing their FERPA training or forfeiting their assistantship.  

    WWAMI Students: Under Coverage Period, enter monthly only or annual if the coverage period does not coincide with the listed Dates of Coverage. You must enter beginning and end dates of monthly coverage e.g. 10/1/2017 to 6/30/2018. If you choose semesters, e.g. enrollment  for coverage during Fall and Spring semesters, United Healthcare requires that you submit two enrollment forms.

  • Graduate Assistant Workload Waiver Request Form

    Graduate Student Employee Waiver Form

    Required when requesting a waiver of the UAA student employment guidelines in allowing a graduate student  who has been awarded an assistantship to work more than 20 hours per week. International students on F-1 or J-1 visas are not permitted to work more than 20 hours per week and therefore are not eligible for this waiver. This form in not needed during summer employment. Do not use this form for GA's having outside employment during their contract. 

  • Advancement to Candidacy 

    Advancement to Candidacy (Master’s - Anthropology and Mechanical Engineering) See your program advisor for information.

    Due at least one semester before the semester in which you intend to graduate. Remember to update your GSP in DegreeWorks to match your candidacy form.

    Advancement to Candidacy (Psychology PhD) See PhD program coordinator for information and forms.

    Per the UAA catalog, doctoral students in some programs may not advance to candidacy until they have passed written and/or qualifying exams. Due at least one semester before the semester in which you intend to graduate. Remember to update your GSP in DegreeWorks to match your candidacy form.

  • Application toGraduate

    Application to Graduate

    Students must apply to graduate to have their degree awarded. Students must be formally admitted to a graduate degree or certificate program to be eligible to graduate. Login to UAOnline and click on Student Services and Account Information. Click on Student Records and select “Apply to Graduate”.  

Graduating Students Exit Survey

The UAA Graduate School is interested in your experiences as a graduate student. Our goal is to use this feedback to assess and improve the quality of graduate education at UAA. Because student input is a vital part of our program evaluation process, graduating students are strongly encouraged to complete this anonymous exit survey. (If your program has its own exit survey, please complete your program survey in addition to this one.) The results of this survey contribute to the overall program assessment we have established to maintain the quality of our graduate programs.
