Fall 2015: Complete smoking and tobacco cessation survey for chance to win Varsity Sports Grill gift card

Smoke and Tobacco-Free UAA is interested in how to best support our faculty and staff following the implementation of the Smoke and Tobacco-Free policy on our campus.

We are seeking current tobacco users (cigarette, chew, etc.) and those who use electronic cigarettes and other similar devices to answer a few questions. If you would, please take a few minutes and complete our brief, eight-question survey.

At the end of the survey, participants will have an option to be entered in a drawing for a $25 gift card to the Varsity Sports Grill. Take the survey.
