Appointment Requests for the Anchorage Campus

Please fill out the provided form to request an appointment to take your University of Alaska Distance proctored exam.  Please note that all exams taken with our office must be for UA Distance courses.  All other exams needing to be taken will be redirected to Advising and Testing with UAA.  

All appointments requested will need to go through an approval process before the actual appointment is granted.  You will receive notification via email stating whether your appointment has been made or if further information is needed.  DO NOT assume that you have an appointment if you have not heard from our office.  

We will respond to your request within 1 business day to confirm or deny your appointment.  If you are trying to schedule an appointment to be taken within the next business please call our office at 907-786-4646 Opt 3 immediately.

When requesting your appointment please note that our office does not process exams that are completed on Fridays until the following business day (Monday).  If your exam needs to be submitted to the instructor by a certain date, please make sure to consider that when scheduling your exam.

Testing Hours are Monday Through Friday from 8am until 5pm 
  • (Extended Hours Testing for the Spring 2012 semester will be on Wednesdays and Thursdays until 7:30pm)

Enter your student ID number. If you don't know what your ID number is, go to me.uaa.alaska.edu to find out.

Enter a phone number that's best for contacting you. Please use dashes (ex. 907-786-4646)

Enter your UA Username.

Enter your UA Email.

Enter a description of your course (for example, ACCT A101).

Enter your 5-digit Course Reference Number (CRN) for the course you are taking.

Enter the number of your exam.

Enter the name of your instructor.

Please select the campus that the course originates from.

Select a date for which you would like to make your exam appointment. Appointments requested less than one business day in advance will be denied.

Please indicate the time you would like to start your exam (AM/PM & 30 minute intervals) Testing starts at 8am and is finished promptly at 5pm. After hours testing is offered until 7:30pm on Wednesdays and Thursdays for the Spring 2012 semester.
