Appointment Requests for the Anchorage Campus

Please fill out the Appointment Request form provided to take your proctored exam(s) at the Distance Education Services office on the UAA-Main Campus. Please note that all exams taken with our office must be for UA Distance courses. All other exams will be redirected to Advising and Testing with UAA.

All appointment requests will need to go through an approval process before the being granted. You will receive notification via email stating whether your appointment has been confirmed or if further information is needed. DO NOT assume that you have an appointment if you have not heard from our office.

We will respond to your request within one business day either to confirm or deny your appointment. If you are requesting an appointment to be taken within the next business please call our office at 907-786-4646 Opt 3 immediately.

Please note that our office agrees to return completed exams to instructors within two business days of the completion date. If your exam has a time sensitive date, please plan accordingly to ensure there is enough time for processing.

Testing Hours are M-F from 8am until 5pm, all testing is by appointment only

Due to limited testing space we do not offer same day testing, please schedule all of your appointments in advance.

	Enter your student ID number. If you don't know what your ID number is, go to me.uaa.alaska.edu to find out.
Phone Number:
	Enter a phone number that's best for contacting you. Please use dashes (ex. 907-786-4646)
UA Username:
	Enter your UA Username.
UA Email:
	Enter your UA Email.
Course Description:
	Enter a description of your course (for example, ACCT A101).
CRN Number:
	Enter your 5-digit Course Reference Number (CRN) for the course you are taking.
Exam Number:
	Enter the number of your exam.
Instructor Name:
	Enter the name of your instructor.
Campus:
	Please select the campus that the course originates from.
Appointment Date:
	Select a date for which you would like to make your exam appointment. Appointments requested less than one business day in advance will be denied.
Appointment_Time:
	Please indicate the time you would like to start your exam (AM/PM & 30 minute intervals) Testing starts at 8am and is finished promptly at 5pm. After hours testing is offered until 7:30pm on Thursdays for the Summer 2013 semester.
Rescheduling Appointment:
	If you are rescheduling an appointment please select Yes
