Immunization RecordsAll residents, prior to check-in, must provide the Housing office with evidence that they have current immunizations. This form must be signed by a licensed Health Care provider. We do not need a copy of your shot rexcords, only this form will be accepted. You will not be allowed access to your room until you have completed immunizatoin requirements turned into our office. |
