Below is a list of forms, documents and resource links that Music Students may find useful.
There are a variety of different forms and documents you may use during your time as a student in the UAA Department of Music. The below list of forms and documents include those originating from the Division of
Performing & Fine Arts and the Department of Music. These documents have been organized in alphabetical order. If you have any questions about these forms, please contact the Performing and Fine Arts Student Information Office, or your Academic Advisor.
For all other University forms, such as Add/Drop, Change of Major/Minor, and Academic Petition forms, please visit the forms page on the Registrar's Office website.
All forms are in PDF format. If you do not have Adobe Reader, please install the program from Adobe's website. To be able to save filled out forms in Adobe Reader, you will need Adobe Reader 8 or later.
