Obtaining Letters of Recommendation

If you need a letter of recommendation from a faculty member for financial aid, job applications, applications to graduate or law schools, or other purposes please see the forms below. These forms can be filled out online and printed out. 

You will need to complete the following for each faculty member from whom you are requesting a letter of recommendation:

Request for Letter of Recommendation Form
Student Reference Request, FERPA Release, and Release of Liability Form

Submit completed forms (including additional information) to the Justice Center.
