What shapes the effectiveness of decision making during crises and hazardous situations?  The recent Deepwater Horizon disaster and the earthquakes, tsunamis, and nuclear accidents in Japan highlight the need for agencies to respond to the unexpected.  Dr. Karlene Roberts has conducted 20 years of research on the design and management of high reliability organizations which identify and avoid errors that can have catastrophic consequences. Roberts is a professor emeritus from the Haas School of Business at UC Berkeley.
