Information about the Health and Injury Survey

Thank you for your interest in the UAA Health and Injury Survey!

Here’s some information about the survey that you may find helpful:

The UAA College of Health is conducting a confidential statewide health and injury survey. To ensure representation from communities across Alaska, addresses were randomly selected to participate.

While we appreciate the interest of all Alaskans, eligibility is limited to households that received an invitation by mail.

 

How do I know if my household was selected?

If your household was selected, you may have received an invitation in the mail, either a letter or a postcard, from the UAA College of Health. Did you receive one of these?

If so, please keep your invitation and do not discard it. Instructions for participating in and completing the survey are provided with each invitation. 

 

I was selected, but why should I participate?

  • Your voice will help inform policy and resources that impact health and injury services in Alaska.
  • As a thank-you for sharing your experiences, you may be eligible to receive $10-$35 in gift cards for completing the survey.

 

What if I’m not eligible to participate?

  • If your address wasn’t selected, you can still support this important effort by spreading the word that the UAA Health and Injury Survey is legitimate and valuable to communities across Alaska.
  • If you received an invitation and you are not an eligible adult, you may share the survey with an eligible adult in your household.
