• VA Report of Change Form: Complete this form if you have:
  
  • changed your major
  • declared a minor
  • changed the type of VA benefits you are utilizing
• Military Rate Request Form: Current active-duty service members, reserves, and national guard members are eligible for tuition to be capped at $250 per credit maximum and have all student fees waived. Please note: The tuition cap does not apply to graduate tuition. The fee waiver does not apply to course or lab fees. The tuition cap and student fees waiver does not apply to dependents.
