Survey to gauge interest in on-site employee health clinic

Dear UAA Employees,

Congratulations on making it through another semester! There is an effort on campus examining the feasibility of creating a comprehensive health clinic for all university employees (staff and faculty). We have created a short survey to gauge overall interest as well as what services we should provide. We appreciate your time and your opinion on this.
