REMINDER: Please use Origami to report incidents that occur on campus

Web-based reporting of injuries and accidents replaced the use of paper forms system-wide back in 2014. UA employees and students should report any incidents that occur on campus by logging onto Origami, our online incident reporting portal, using their UA ID # or UA username and password. Incidents that should be reported through this system include:

• Employee Injuries • Non-Employee Injuries (including students and visitors) • Damaged Property • Damaged Vehicles • Unsafe Conditions
