Senior Nursing Students' Survey

We are a group of senior nursing students conducting a survey for COVID-19 vaccine awareness in the UAA campus population. The University of Alaska Anchorage School of Nursing senior nursing students are completing an academic assessment project about COVID-19 vaccination awareness in the community. There has been an Institutional Review Board Determination that this is considered to be non-research. This information will be used to develop a community awareness campaign using public service announcements.

Your participation is voluntary and your responses are confidential. This survey should take no more than 5 minutes to complete. Your participation in the survey, indicates your consent.
